In the case of emergency, or even when visiting a new doctor - a completed medication record assures that health care providers have accurate information regarding medications you are taking. Keep a copy of this record in your wallet, car and at home.
Download a medication record and complete with the following information:
- List every medication including prescriptions, over-the-counter products (like aspirin), vitamins, herbs and natural remedies.
- List dose amount and frequency, how long you've been taking the medicine, why you take the medicine and the prescribing physician.
- List your pharmacy, primary care physician and emergency contact.
