Home Care and Hospice

Home care and hospice representatives work directly with physicians, case managers, and unit staff in coordinating post-hospital care. Services available include direct care by registered nurses, personal assistance by certified nurse aides, rehabilitative care of physical, occupational, and speech therapists, as well as help from social workers. Infusion therapy, oxygen, and durable medical equipment are also available.
