Not only is it easy to do, but chances are your doctor will be very supportive of your decision to enroll. Simply:

• Ask your doctor to fill out the Physician Referral Form and fax it to our office at 303-269-4504. You may call 303-269-4500 to schedule your appointment after your doctor has faxed the order form. If you have any problems, please feel free to contact us at 303-269-4500 and we will fax the form directly to your doctor.
• The form includes your diagnosis, recent lab tests and results, and current medications. This will help us begin to put together a program that's right for you.
• Once we receive the completed form from your doctor, we will call you within five days to set up an initial assessment. And then you begin!
